Visitor's name and birthdate
Visitor's travel dates (arrival in Canada and departure from Canada to return to home)
Your BCAA Membership number (if applicable)*

Applicable to All BCAA Travel Insurance
There is no coverage for any trips taken to Russia, Ukraine, Belarus or any other country subject to the Level 3 (Avoid non-essential travel) and Level 4 (Avoid all travel) travel advisories issued by the Canadian government due to the armed conflict with Russia, regardless of the war and whether the war is directly or indirectly involved in losses incurred. This will apply as soon as you enter the impacted countries and until you return to your home province.
Applicable to Trip Cancellation & Interruption Coverage:
There is no coverage for any Trip Cancellation or Trip Interruption claims related to COVID-19 (Coronavirus). Coverage for claims not related to COVID-19 may still be available. Learn more at bcaa.com/coronavirus.
Applicable to Visitors to Canada Travel Emergency Medical Insurance:
BCAA Visitors to Canada Travel Emergency Medical Insurance provides coverage for COVID-19 related medical expenses, up to the coverage limit you select. Coverage limit for COVID-19 related emergency medical expenses applies and is based on the coverage amount selected at time of purchase. You must be fully vaccinated for COVID-19 (including all recommended vaccines, including booster shots made available to you) and vaccine dose(s) must have been administered at least two weeks prior to the effective date, arrival date or COVID-19 diagnosis date, whichever occurs later. Children under the age of 18 are covered regardless of their vaccine status.
